Intake Process, Client Forms, Fees, & Insurance
Maria Droste Counseling Services remains committed to providing accessible, affordable, and timely treatment to regardless of insurance status. Our agency accepts mental health counseling and holistic therapy referrals for any person who has no health insurance, as well as those who have state or private insurance.
Intake Process
If you would like to become a new client, please call us at (617) 471-5686. One of our staff will be happy to speak to you and gather your contact information, your circumstances, and the service you are requesting. If we are unavailable when you call, leave your name and phone number, and we will return your call as soon as we are able. Typically within two weeks of when the initial intake process was completed, a therapist who is best suited for your needs and availability will call you back to arrange your first session together. If your insurance is accepted at our agency and you plan to use it with us, please be prepared with your insurance information when you call. If you have no insurance or if full payment would be a hardship, you are invited to use our sliding scale which can make our services affordable. Further information about this service will be discussed during your intake.
New Clients with Confirmed Appointments
If you have your first appointment scheduled with your therapist, please complete the following paperwork prior to your visit. You must complete the following form that is relevant to you:
- New Client History & Notice of Privacy Practices form, either for Adult OR Youth (under age 18) based on what is most relevant to you.
- The Notice of Privacy Practices form. This form is to be completed if you are filling out a YOUTH history form to accompany the YOUTH history form.
If you are coming for couples therapy, both partners should complete their own paperwork.
NOTE: If you have not yet called our agency and completed a brief intake with one of our staff members, PLEASE DO NOT complete the forms below. Only once your therapist has confirmed your first appointment will you be expected to complete these forms.
Fee Structure
Our Mission is to help ALL in need who can benefit from our services. We are committed to negotiating a fee based on an individual’s needs. We accept several insurances and help numerous clients afford our services privately without insurance through our sliding scale fee agreement.
